It is essential that you become familiar with your insurance policy prior to scheduling a new patient appointment. Please review the handbook provided to you by your insurance provider. Review the description of the benefits they provide. Call Member Services at your insurance provider with any questions you may have. Many insurance companies provide some coverage for infertility work up, diagnosis and treatment of underlying conditions. If you have two insurance providers, be careful to research your benefits with each provider. Approximately 70% of patients treated at RBA receive some reimbursement for medical charges. Be sure to bring your insurance card with you to all visits and notify our business office of any changes in your coverage.
The following is a list of insurance plans with which we are contracted:**
- Aetna HMO with referral
- Aetna POS and USAccess with referral
- Aetna PPO and Traditional Choice Indemnity
- Aetna Elect Choice and Managed Choice with referral
- BC/BS of Georgia POS and PPO, NOT HMO
- BC/BS Out-of-State PPO and POS, NOT HMO
- First Health
- Kaiser (with infertility treatment coverage)
- Principal Southcare
- Multiplan
- Private Healthcare System (PHCS)
- United HealthCare (all types)
Patients with IVF coverage through other insurance providers will be required to pay the deposit for the IVF cycle in advance. You will receive encounter forms for each visit that you may then file for reimbursement from your insurance provider.
Blue Cross/Blue Shield (BC/BS): We file for out of network benefits only. Patients may be susceptible to a deductible and copy/coinsurance %.
We do not accept Medicare, Champus/Tricare or federally-funded plan patients.
